These forms are available:
- Petitions for exportation to Energy Solutions (Utah) 2024
- Petitions for exportation to Waste Control Specialists (Texas) 2024
- Disposal Report Form-To be submitted for EACH Petition issued
NOW AVAILABLE-Year 2024 petition forms are for waste shipments between November 1, 2023 thru December 31, 2024.
Be sure to sign your petition. TO HELP SAVE TIME-You can E-mail a copy of your completed Petition along with a copy of your Check, prior to mailing. I can begin the approval process sooner. Mail the completed form with the appropriate fee to the Southwestern Low-Level Radioactive Waste Compact Commission, 1731 Howe Ave. #611, Sacramento CA 95825. If you have any difficulty, contact the Executive Director at swllrwcc@swllrwcc.org Please issue the check to: Southwestern LLRW Commission

The Compact’s tax identification number is 68-0289659. Please call and request a W-9 if necessary.
The approved petition that you will receive will show an “assigned name & code”. Be sure to use it with the generator’s name on all manifests.
A disposal report is to be submitted for each petition and is due no later than June 30 of the following year of the petition’s year date. The Compact will not allow a new petition to be approved if a disposal report is overdue. Follow the same procedures as described above to complete and print a disposal report, sign it and Email to kathydavis@swllrwcc.org
